I created a form and he is now a constant! I can not delete it or replace it with another form, I deleted all the html codes and even then it appears in the preview appears and when I put the widgets, but widgets does not appear in the colors of the form, but appears in the fields. In short: can not erase any way the first form I tried to do. Already disable and enable the plugin and the form problem is always there. Can anyone help me to delete this form you not want to disappear?

Actually you just have to configure the form settings from the available settings and the form appearance changes.
But if you unable to do so then please try once by disabling and removing all the plugin files along with the plugin database table.
Then install a fresh copy of the plugin and try again.

2) deleted the plugin directory "maxblogpress-option-form-adder" in / public_html / wp-content / plugins / maxblogpress-optin-form-adder.
3) reinstalled the plugin
4) activated again
The problem was not resolved.
Please, what is the name of the database plugin "maxblogpress-option-form-adder"?
I want to delete this file exists in the database and repeat my actions to 1) up to 4) above.

The table name is "wp_ofa_form_options" wp_ depends on the prefix you have used.
