Detecting the form in optin form settings

I pasted my autoresponder code under "Optin form settings > Copy and Paste your optin form code:"
Under optin form placement, i want to use sidebar widget. the form is already showing on my website sidebar, but when i try to subscribe, i don't get the contact details in my autoresponder. i think there's a problem with "detecting the form" step. it's not reading my optin form code. after selecting "detect form" it said select the name/email field and "if you are unsure about the name/email field then post your optin form code in our forum...we'll check your code and will suggest the name/email field." that is why i am in this forum. what's the name and email field?
